What documentation is needed?

Answer

Students must provide the Office of Disability Services with professional documentation of their disability. Documentation requirements vary according to the unique and individual nature of each student's disability.

To provide professional documentation to the Office of Disability Services, please follow these instructions: 

  1. Log into Accommodate.
  2. Select Accommodation on the left sidebar. 
  3. Select Documents from the dropdown menu that appears.
  4. Select the blue box that states Add New Accessibility Document.
  5. Complete the form and upload the necessary documentation. 
  6. Click Submit.

Other options for providing your professional documentation later include: 

  • Have your current provider of care fax your documentation to our office at 605.688.4504 (private fax).
  • Hand-deliver your documentation to the Office of Disability Services:
